Terms & Conditions for SwimSafer Test

1. Compulsory things to do:

1.1. Must submit hard copy of the indemnity form on the day of test

1.2. Must bring extra clothing as instructed in the email required for the test

1.3. In the event either or both of the above task are not completed, Aquarian reserves the right not to allow your child to participate in the test. Any request for refund will not be entertained.

2. I am aware of the risks involved in the SwimSafer Test and hereby confirm my child’s/ward’s participation in this test. I shall not hold Aquarian Aquatic School and its SwimSafer Instructors or Officers responsible or liable for any death, personal injury, loss or damage that my child/ ward *may suffer or incur with this test.

3.  I understand that signing up for this test does not guarantee a pass.

4. Depending on the number of participants in each stage, we may have to reschedule or combine the groups. We require a minimum of 6 participants per test.

5. Aquarian reserves the right to reject or reschedule any registration due to overwhelming response. Participants affected will be informed.

6. I shall be responsible to inform the SwimSafer Instructor/Tester if my child is unwell or taking medication

7. I have checked and confirmed that all information provided in this form is correct.

8. For any typographical error incurred, I agreed to the admin charge of $5 per amendment of typo imposed by SwimSafer/SwimSafer Secretariat and $5 for re-printing of certificate, where applicable.

9. Please be reminded to complete the theory test within 1 week from the practical test (3 May 2020). Failure to do so may delay the processing of your child's certificate.
